Description: Community support workers provide ongoing, individualized, 1-on-1 support to persons with serious and persistent mental illness. Support provided through a continuum of case management functions including assessment, linkage to resources, person-centred planning; counselling, skill development, symptom management, advocacy, family support and public education. Provides support and education. Service is available throughout Essex County with satellite offices in Leamington and Essex
