Description: A facilitator can provide assistance to Ontario Works recipients who have or suspect they have disabilities in gathering documentation such as psychological/psychiatric assessment reports, treatment reports, medical records, and/or school transcripts. This documentation can then be used to assist clients in applying for Ontario Disability Support Program – Income Supports, developing a return to school plan, determining appropriate workplace accommodations, or seeking appropriate medical treatment.
